Attached below is the official Wyld Chyld Piercing Form. Please be sure to read over it carefully.
*For any person under 18 looking to get a piercing, New York State law requires a copy of our piercing form notarized, a parent or legal guardian on the premises and a copy of their ID. We do not pierce anyone under the age of 12.
